Assessments That Answer Your Questions

ANNUAL WELLNESS SCREENING

The wellness screening is similar to seeing a primary care doctor for an annual checkup with a focus on prevention and early detection. This brief evaluation includes a screening for anxiety and depression and serves as a general mental health check up. In addition, the wellness screening provides recommendations to maintain and enhance your or your child’s wellbeing.

 

PARENT-CHILD RELATIONSHIP ASSESSMENTS

Parent-child relationship assessments examine the quality and nature of the relationship between parents and their children under 12 years old. These assessments are designed to help parents improve their relationships with their children. Parent-child relationship assessments involve a set of tasks that are video recorded, which help determine strengths and challenges in the interactions. Parent-child relationship assessments include a video recorded parent-child relationship assessment session and a one-hour feedback session to review findings.

 

DIAGNOSTIC EVALUATION

Diagnostic evaluations help individuals, the school, the doctor, or other support people gain clarification about what is causing symptoms in the child, address concerns about their behavior, mood or social interactions, and lead to recommendations. These evaluations vary in complexity as they are tailored to answer specific questions. Diagnostic evaluations may include parent interviews; a child interview; questionnaires completed by the child, parents, and teachers; interactive tests involving various activities and a review of records. A written report is provided with diagnostic impressions and individualized recommendations and reviewed during a one hour feedback session.

 

ATTENTION DEFICIT HYPERACTIVITY DISORDER EVALUATION

This evaluation can help confirm ADHD diagnosis and/or determine if there are other causes for the symptoms. The ADHD evaluation includes parent interviews, child interview, behavioral analysis through checklists completed by parents and teachers, computerized attention assessment, review of records, a written report with a diagnosis when appropriate and a list of recommendations. This evaluation includes a one hour feedback session to review findings.

 

SCHOOL SUCCESS CONSULTATIONS

This evaluation is specifically designed for children who are not yet achieving their potential at school due to emotional or behavioral issues. This evaluation may be appropriate if a child is regularly demonstrating disruptive, withdrawn, or other problematic behaviors at school. A clinician from Intuition Wellness Center can provide a school observation, interview parents and teachers, behavioral checklists completed by parents and teachers, write a thorough report that includes detailed recommendations and deliver one-time or ongoing consultation to school personnel.

 

ACADEMIC EVALUATION

Academic evaluations pinpoint educational strengths and weakness of children or teens and identify at what grade level they are performing. This evaluation may be appropriate if a child or teen is struggling with some or all of their academic courses or possibly failing their classes. A clinician from Intuition Wellness Center can provide an academic evaluation and write a report that includes detailed recommendations.

 

COGNITIVE EVALUATION

This type of evaluation identifies intellectual strengths and weaknesses and corresponding recommendations. This evaluation may be appropriate if you want to determine a child or teen’s IQ or if you suspect they are struggling with memory, verbal comprehension, processing speed or reasoning. A clinician from Intuition Wellness Center can provide a cognitive assessment and write a thorough report that includes detailed recommendations.

 

PERSONALITY EVALUATION

Personality evaluations are specifically designed to gain insight into aspects of individual character that influence behaviors and relationships with others. This evaluation may be appropriate if a child or teen is displaying disruptive, withdrawn, manipulative or otherwise problematic behaviors at home, school, or in relationships. A clinician from Intuition Wellness Center can provide a personality assessment and write a thorough report that includes detailed recommendations.

 

COMPREHENSIVE EVALUATION

Comprehensive evaluations are combinations of the above mentioned assessments. This evaluation may be appropriate if a child or teen is having behavioral, intellectual, academic, or relational problems related to their personality. A clinician from Intuition Wellness Center can provide a comprehensive evaluation, an  interview with parents and teachers, complete cognitive, academic, and personality testing, with a thorough report that includes detailed recommendations and one-time or ongoing consultation to school personnel or others as needed.

 

PLEASE NOTE
  • An intake is typically required before a determination can be made as to the most helpful and appropriate assessment.
  • Assessments often require multiple sessions after the intake to gain a thorough picture; 1-3 sessions of varying length is typical for administration of the assessment tools; an in-person feedback session will be scheduled to review results.
  • A retainer for 50% of the total is expected at the time of the initial testing appointment. The remaining balance is expected when results and a final report are received.
  • Fees are based on time spent conducting a thorough assessment.
  • Portions of this service may be billable to insurance when provided by a licensed clinician. Thorough assessments and reports are not typically fully covered by insurance.
  • Our licensed clinicians at Intuition Wellness Center are credentialed with BCBSAZ & Aetna.
  • A detailed statement (superbill) of fees and services will be provided by request.
